NEW YORK, April 21, 2020 /PRNewswire/ -- Immunic, Inc. (Nasdaq: IMUX), a clinical-stage biopharmaceutical company focused on developing best-in-class, oral therapies for the treatment of chronic inflammatory and autoimmune diseases, today reported that its lead asset, IMU-838, a selective oral DHODH inhibitor, has successfully demonstrated preclinical activity against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). More specifically, IMU-838 was observed to inhibit replication of clinical isolates of SARS-CoV-2 associated with coronavirus disease 2019 (COVID-19). In cellular assays, IMU-838 demonstrated this antiviral activity at concentrations which are well below the blood concentrations associated with IMU-838 dosing regimens studied in ongoing and previous clinical trials. These positive results have encouraged Immunic to prepare a clinical development program for IMU-838 as a potential treatment option for patients with COVID-19 and potential other, future viral pandemics.

"While most such efforts are focused on drugs and vaccines aimed at viral targets, it is particularly important to explore treatment options targeting host cell factors that are able to act with less dependence on the genetic drift of viruses and synergistically to standard-of-care antiviral therapies. With that in mind, DHODH inhibitors, such as IMU-838, present a very promising approach."

Prof. Vehreschild went on to note that, "DHODH inhibition selectively blocks the de novo production of pyrimidines, an essential RNA building block, in metabolically activated cells such as virus-infected cells. In addition, DHODH inhibitors may help reduce the severity or virulence of infection through several mechanisms. First, DHODH inhibition prevents the production of viral RNA and proteins and, therefore, prevents viral replication. Second, it induces innate immunity in an interferon independent setting as an early host-based antiviral response. Third, DHODH inhibition may ameliorate the overshooting immune response, as seen in severe COVID-19 cases, by selectively targeting highly activated immune cells, but without broader anti-proliferative or immunosuppressive effect."

IMU-838 is already being investigated in ongoing phase 2 clinical trials in patients with relapsing-remitting multiple sclerosis, ulcerative colitis and primary sclerosing cholangitis. Although the drug is being studied in these ongoing trials primarily for its anti-inflammatory effect, one of IMU-838's postulated benefits is a host-based antiviral effect, which may be important in these indications to potentially prevent virus reactivations known to occur with other immunomodulatory therapies. In support, IMU-838's antiviral activity has previously been demonstrated in vitro against human immunodeficiency virus (HIV), hepatitis C virus (HCV), human cytomegalovirus (hCMV), Arenavirus and Influenza A virus. Given what is known about the natural course of the disease, IMU-838's combination of antiviral activity against the highly pathogenic SARS-CoV-2 and a selective immunomodulatory effect against highly activated immune cells may be a promising profile for the treatment of COVID-19. Importantly, IMU-838 has an attractive pharmacokinetic, safety and tolerability profile and, to date, has already been tested in about 650 individuals.

Management noted that Immunic is collaborating with several regulatory agencies and other institutions in the United States and in Europe to define and accelerate the development path for IMU-838 in COVID-19. The aim is to investigate IMU-838 as an oral treatment option for COVID-19 and to enable the use of IMU-838 in treating current and potential future pandemic threats. Immunic intends to initiate a prospective, multicenter, randomized, placebo-controlled, double-blind phase 2 clinical trial in patients with moderate COVID-19 disease and clinical symptoms, in order to evaluate efficacy, safety and tolerability. The plan is to test IMU-838 versus placebo on the background of investigator's choice of standard-of-care therapy used in both treatment arms. Adequate drug supply exists to begin clinical testing in COVID-19 very soon.

Die 4SC AG (4SC, FWB Prime Standard: VSC) hat ihr Immunologie-Portfolio bestehend aus zwei Entwicklungsprogrammen einschließlich Schutzrechte und Know-how an die Immunic AG (Immunic) veräußert. Einen entsprechenden Vertrag haben die beiden Unternehmen jetzt unterzeichnet. Immunic führt sämtliche Forschungs- und Entwicklungsaktivitäten im Zusammenhang mit den Medikamentenprogrammen fort. Kann es sein, dass viele 4SC verkaufen oder nicht kaufen, weil sie jetzt lieber in Biotech Firmen investieren, die etwas mit der Corona Phantasie zu tun haben?

Planegg-Martinsried (pta/17.09.2020/07:30) Die 4SC AG (4SC, FSE Prime Standard: VSC) gab heute bekannt, dass ein Trial in Progress-Poster mit dem Titel: 1153TiP - A phase II, open label study to investigate the efficacy and safety of domatinostat in combination with avelumab (BAVENCIO®) in patients with advanced unresectable/metastatic Merkel Cell Carcinoma (MCC) progressing on anti-PD-(L)1 antibody therapy - the MERKLIN 2 study - auf dem European Society for Medical Oncology (ESMO) Virtual Congress präsentiert wird. Avelumab wird von Merck KGaA, Darmstadt, Deutschland, und Pfizer Inc. gemeinsam entwickelt und vermarktet. Das Poster wird bis zum 21. September auf dem ESMO Virtual Congress und danach auf der 4SC Website zugänglich sein.
"Die MERKLIN 2-Studie adressiert einen hohen medizinischen Bedarf bei Patienten mit fortgeschrittenem, inoperablen Merkelzellkarzinom (MCC), die unter Anti-PD-L1 oder Anti-PD-1-Monotherapie fortschreiten. Diese Patienten haben keine wirklich etablierte Therapieoption, schon gar keine dauerhaft wirksame und brauchen definitiv zukünftig bessere Behandlungsmöglichkeiten. Die Kombination von Domatinostat und Avelumab hat eine klare und solide wissenschaftliche Rationale, indem Domatinostat auf gut beschriebene Mechanismen bei MCC abzielt einer effektiven Immunantwort zu entgehen, und um hierdurch eine Synergie mit der Checkpoint-Inhibition zu erzielen. Die MERKLIN 2-Studie untersucht die Wirksamkeit und Sicherheit der Kombination bei diesen Patienten. Über 40 Zentren in Europa und den USA sind geplant, um Patienten in die Studie einzuschließen, und ich freue mich, das Trial in Progress-Poster für MERKLIN 2 beim diesjährigen virtuellen ESMO-Kongress im Namen meiner Kollegen zu präsentieren, um ein Update zu geben und diese wichtige Studie für die Teilnahme und mögliche Zuweisung von Patienten an die teilnehmenden Studienzentren bekannt zu machen. Über Domatinostat
Domatinostat ist ein oral verabreichter, niedermolekularer, Klasse-I-spezifischer HDAC-Inhibitor. Domatinostat verstärkt die körpereigene Immunantwort gegen Krebs. Das Tumor- und das den Tumor umgebende Gewebe wird durch Domatinostat verändert, so dass der Tumor für das Immunsystem besser sichtbar wird. Immunzellen können damit leichter in den Tumor einwandern und diesen bekämpfen.
Domatinostat wurde in einer Phase-I-Studie mit 24 intensiv vorbehandelten Patienten mit verschiedenen weit fortgeschrittenen Blutkrebsarten untersucht und hat sich als gut verträglich erwiesen. Mit einer 28 Monate anhaltenden vollständigen und einer 8 Monate anhaltenden teilweisen Remission konnten vielversprechende Anzeichen auf Wirksamkeit beobachtet werden.
Neben dem Potenzial von Domatinostat als Monotherapie bei Krebserkrankungen evaluiert 4SC Möglichkeiten verschiedener Kombinationstherapien, insbesondere im Bereich der Immunonkologie. Dazu hat 4SC eine Phase-Ib/II-Studie mit Domatinostat in Kombination mit dem anti-PD-1 Antikörper Pembrolizumab (einem sogenannten Checkpoint-Inhibitor) bei Patienten mit fortgeschrittenem Melanom gestartet - die Daten des ersten Teils der Studie wurden auf dem ESMO Kongress 2019 präsentiert. Eine zweite Phase-II-Studie mit Domatinostat in Kombination mit dem anti-PD-L1 Antikörper Avelumab (ebenfalls einem Checkpoint-Inhibitor) wird zurzeit von Prof. David Cunningham am The Royal Marsden NHS Foundation Trust (London, Vereinigtes Königreich) in Patienten mit fortgeschrittenem, Mikrosatelliten-stabilem Magen-Darm-Krebs durchgeführt. Und in Zusammenarbeit mit dem Netherlands Cancer Institute in Amsterdam wird in der DONIMI-Studie, eine multizentrische, Prüfarzt-initiierte Phase-lb-Studie, die neoadjuvante Kombination von Domatinostat, Nivolumab und Ipilimumab beim Hochrisiko-Stadium III Melanom getestet.
Avelumab - Zugelassene Indikationen in den USA
Avelumab (BAVENCIO®) ist in den USA für die Erhaltungstherapie von Patienten mit lokal fortgeschrittenem oder metastasiertem Urothelkarzinom (UC) indiziert, bei denen die Erstlinien-Chemotherapie mit platinhaltiger Chemotherapie nicht weiter fortgeschritten ist. BAVENCIO ist auch für die Behandlung von Patienten mit lokal fortgeschrittenem oder metastasierendem UC indiziert, bei denen die Krankheit während oder nach einer platinhaltigen Chemotherapie fortschreitet oder die innerhalb von 12 Monaten nach einer neoadjuvanten oder adjuvanten Behandlung mit platinhaltiger Chemotherapie ein Fortschreiten der Krankheit aufweisen.
In den USA ist BAVENCIO in Kombination mit Axitinib zur Erstlinienbehandlung von Patienten mit fortgeschrittenem Nierenzellkarzinom (RCC) indiziert.
In den USA erteilte die FDA eine beschleunigte Zulassung für BAVENCIO für die Behandlung von Erwachsenen und pädiatrischen Patienten ab 12 Jahren mit metastasiertem Merkelzell-Karzinom (MCC). Diese Indikation ist im Rahmen der beschleunigten Zulassung auf Basis der Tumoransprechrate und der Ansprechdauer zugelassen. Die Aufrechterhaltung der Zulassung kann vom Nachweis und der Beschreibung des klinischen Nutzens im Rahmen von konfirmatorischen Studien abhängig sein.
Avelumab - Wichtige Sicherheitsinformationen aus von der US-FDA-zugelassenen Produktinformation
Die Warnhinweise und Vorsichtsmaßnahmen für Avelumab (BAVENCIO®) beinhalten immunvermittelte Nebenwirkungen (wie Pneumonitis und Hepatitis [einschließlich letaler Fälle], Colitis, Endokrinopathien, Nephritis sowie andere immunvermittelte Nebenwirkungen als Einzelwirkstoff oder in Kombination mit Axitinib [die schwerwiegend sein können und letale Fälle einschließen]), infusionsbedingte Reaktionen, Hepatotoxizität in Kombination mit Axitinib, wichtige unerwünschte kardiovaskuläre Ereignisse (MACE) in Kombination mit Axitinib [die schwerwiegend sein können und letale Fälle einschließen] und Embryo- / Fetotoxizität.
Zu den häufigen (bei mindestens 20% der Patienten berichtet) Nebenwirkungen bei Patienten, die mit der BAVENCIO®-Monotherapie behandelt werden, gehören Müdigkeit, Schmerzen des Bewegungsapparats, Diarrhö, Übelkeit, infusionsbedingte Reaktionen, periphere Ödeme, verminderter Appetit, Harnwegsinfektionen und Hautausschlag. Zu den häufigen (bei mindestens 20% der Patienten berichtet) Nebenwirkungen bei Patienten, die BAVENCIO® in Kombination mit Axitinib erhalten, gehören Diarrhö, Müdigkeit, Bluthochdruck, Schmerzen des Bewegungsapparates, Übelkeit, Mukositis, palmar-plantare Erythrodysästhesie, Dysphonie, verminderter Appetit, Schilddrüsenunterfunktion, Hautausschlag, Hepatotoxizität, Husten, Atemnot, Bauch- und Kopfschmerzen. Bei mindestens 10% der Patienten mit Merkelzellkarzinom, die mit der BAVENCIO®-Monotherapie behandelt wurden, sind bei den hämatologischen Laborwerten der Grade 3-4 Anomalien der Lymphopenie zu verzeichnen; bei Patienten, die BAVENCIO® in Kombination mit Axitinib erhalten, sind bei den klinisch-chemischen Anomalien des Grades 3-4 Bluttriglycerid- und Lipasezunahme zu verzeichnen.
