QLT reiterates confidence in Visudyne's potential

VANCOUVER, Aug. 30 /PRNewswire-FirstCall/ -- QLT (Nachrichten) (NASDAQ: QLTI; TSX: QLT) reiterated its confidence in Visudyne(R)'s potential in the treatment of age-related macular degeneration (AMD). The company also reiterated its 2004 guidance of Visudyne sales of US$430 to $455 million and its post-integration guidance for the combined QLT-Atrix company of a cash EPS compound annual growth rate of 20-25%. This statement is made following the U.S. Food and Drug Administration (FDA) advisory panel for the anti-VEGF aptamer which was held on Friday, August 27th. At this panel, Phase II/III results were presented for the anti-VEGF aptamer, confirming QLT's earlier view of the top- line aptamer data, that those data appear to provide no advantage over Visudyne therapy for patients with AMD, the leading cause of blindness in patients over 50. In the United States, Visudyne is reimbursed by the Centers for Medicare and Medicaid Services (CMS) for certain patients with all three forms of wet AMD, predominantly classic, occult and minimally classic.

"While there are no head-to-head comparative clinical trials of Visudyne vs. the anti-VEGF aptamer, the data presented at the panel did not appear to provide any advantage over Visudyne and requires nine repeated intravitreal injections yearly, directly into the eye," said Paul Hastings, President and Chief Executive Officer of QLT Inc. "Given the discrepancies in efficacy across all subtypes that was evident in the FDA briefing document, we believe that retinal specialists who now for the first time have access to this more detailed data, will take this into consideration in their treatment decisions."

QLT: Konkurrent Eyetech rückt näher

Die Titel des kanadischen Biotech-Unternehmens QLT (Nachrichten) verloren an der Nasdaq heute bislang 8,65 Prozent auf 14,68 Dollar. Zuvor hatte ein Ausschuss der amerikanischen Zulassungsbehörde FDA einen positiven Kommentar zum Zulassungsantrag für das Medikament Macugen abgegeben. Der Wirkstoff wurde von Eyetech Pharmaceuticals (Nachrichten) und Pfizer entwickelt und könnte zu einem harten Konkurrenten für QLTs Präparat Visudyne bei der Behandlung von altersbedingter degenerativer Blindheit werden. Dabei sind Analysten verschiedentlich der Meinung, dass sich Macugen auf dem Markt schwer tun könnte. Das Mittel muss alle 6 Wochen ins Auge eingeführt werden, während Vysudyne lediglich in den Arm gespritzt werden muss.

Zu einer endgültigen Entscheidung konnte sich die FED allerdings noch nicht durchringen. Die brachte auch die Titel von Eyetech unter Druck, die aktuell 8,14 Prozent auf 34,41 Dollar abgeben. Pfizer verlieren 0,92 Prozent auf 32,25 Dollar.

Die Entscheidung der FDA über Macugen wird für den 17.12.2004  erwartet.

NEW YORK, August 30 (New Ratings) - Analysts at McAdams Wright Ragen maintain their "buy" rating on QLT (QLT.TOR). The target price is set to $32.

In a research note published this morning, the analysts mention that the company's Visudyne, a treatment for Age-related Macular Degeneration (AMD), is expected to witness competition in the forthcoming years. The FDA is likely to grant formal approval for Macugen, Eyetech Pharmaceuticals' product for AMD, by the end of this year, the analysts say. QLT's stock is attractively valued at present, according to McAdams Wright Ragen.

Obwohl das FDA-Gremium kein formales Votum im Hinblick auf eine Zulassungsempfehlung von Macugen aussprach, so sprach die Aussagen der Experten durchweg sehr positiv und lassen auf eine Zulassung schließen. Auch Eyetech (NasdaqNM:EYET; WKN: 251716) und sein Pharmapartner Pfizer (NYSE:PFE; WKN: 852009) gaben sich am Freitag nach Beendigung des Treffens optimistisch bezüglich der finalen Zulassung von Macugen noch in diesem Jahr. Die bisherigen Daten, so Dr. Jose Pulido, ein Augenspezialist der Mayo Clinic und Mitglied des Beratergremiums, seien viel versprechend und sprächen für die finale Zulassung.

Wörtlich meinte Dr. Pulido: "Ich bin wirklich beeindruckt von den vorgelegten Daten". Nicht nur Dr. Pulido, auch der Rest des Gremiums vertrat die Meinung, die von Eyetech und Pfizer generierten Daten wären robust genug für eine Zulassung. Der stellvertretende Direktor der Abteilung Augenheilkunde der FDA, Dr. Wiley Chambers, schloss sich dieser Meinung an. Wörtlich meinte Chambers, alle Teilnehmer des Gremiums wären von den Daten überzeugt, die eindeutig darauf hinweisen, dass Macugen eine Wirksamkeit gegen die altersbedingte Maculadegeneration besitzt.

Auch im Hinblick auf die Datenlage äußerte sich das Gremium zugunsten von Eyetech. Sowohl Sicherheits- als auch Wirksamkeitsdaten wären ausreichend, so die Gremiumsmitglieder, um der FDA eine finale Entscheidung zu ermöglichen. Karin Hehenberger, Sprecherin von Eyetech, meinte, das Treffen mit der FDA und dem Expertengremium hätte nicht besser laufen können. Obwohl das Gremium, wegen des Pilot 1 Prioritiy Review Status von Macugen keine finale Empfehlung für oder gegen die Zulassung aussprach, ist man bei Eyetech und auch bei seinem Partner Pfizer von der Zulassung noch in diesem Jahr überzeugt.

Das Pilot 1 Prioritiy Review Programm der FDA ist ein Sonderstatus, der nur Medikamenten verliehen wird, die einen signifikanten klinischen Vorteil gegenüber anderen Behandlungsalternativen demonstrieren konnten. Im Falle der Anwendung dieses Status genehmigt die FDA die Überstellung des Zulassungsantrages in mehreren Teilen und verspricht zudem eine Überprüfung innerhalb von sechs Monaten nach Überstellung des ersten Teilantrages. Die finale Entscheidung der FDA wird also am 17. Dezember fallen.

Obwohl die Zulassung noch nicht 100 Prozent sicher ist, so haben die Äußerungen der Gremiumsmitglieder und der FDA-Verantwortlichen bereits ein Signal für eine Zulassung gegeben. Auch die Tatsache, dass keine weiteren Daten nötig sind und lediglich zusätzliche Phase IV Studien, zur Evaluierung von Langzeitwirkungen und Behandlungsdauer, nach der Zulassung einstimmig befürwortet wurden, spricht schon sehr für eine Zulassung von Macugen.

Karin Hehenberg meinte zudem, auch für QLTs Visudyne hätte es kein formales Votum gegeben, dennoch wurde das Präparat, das von der Schweizer Novartis vermarktet wird und im zweiten Quartal bereits $109,3 Millionen Umsatz erzielen konnte, letztlich von der FDA zur Vermarktung freigegeben. Die Aussagen des Gremiums dürften auch Macugen bald den Weg auf den Markt ebnen. Das Präparat, das alle sechs Wochen direkt in das Auge injiziert wird, hat laut einigen U.S. Analysten ein weltweites Marktvolumen von $1 Milliarde. Für Eyetech, das bisher noch kein Produkt vermarktet, ist Macugen von ganz entscheidender Bedeutung, weshalb eine Zulassungsniederlage, die hoffentlich nicht eintreten wird, zu einem Kurssturz beitragen dürfte. Der gestrige Verlust von dürfte aber eher auf Gewinnmitnahmen zurückzuführen gewesen sein, denn auf Bedenken bezüglich der Zulassung, schließlich ist Eyetech in der letzten Woche um fast 20% geklettert.

Winton Gibbons, Analyst bei William Blair & Co., der dem Meeting beiwohnen durfte, ist der Meinung, das Gremium wäre für eine Zulassung von Macugen gewesen, obwohl dies während der Beratungen nicht direkt artikuliert worden sei. Die Verantwortlichen der FDA hoben hervor, dass ein finales Votum auf Grund des Teststadiums des Programms, unter welchem Macugen die Zulassung erteilt werden könnte, nicht wünschenswert sei. Wegen der teilweisen Überstellung der Daten wäre ein solches Votum noch zu früh, so die FDA. Nach Wirksamkeit und Sicherheit müsse nun erst die Chemie, der Herstellungsprozess und andere Informationen eingehende geprüft werden. Die bereits geprüften Daten sprächen aber für Macugen und der Rest der eingereichten Informationen wäre ausreichend für die weitere Evaluierung der Verbindung.

Macugen wurde zur Behandlung der schwereren Form der Maculadegeneration, der feuchten AMD, entwickelt. Unter feuchter AMD leiden in den USA schätzungsweise 1,2 Mio. Menschen. Die feuchte AMD entsteht, wenn abnorme Blutgefäße im Auge zu lecken beginnen und den Punkt des schärfsten Sehens auf der Netzhaut, die Macula, zerstören. Unbehandelt führt die Erkrankung, die sich durch einen schwarzen Punkt in der Mitte des Sehfeldes bemerkbar macht, meist zur Erblindung.

Macugen, das an 1.186 Patienten getestet wurde, hemmt das Protein VEGF, den vaskulären epithelialen Wachstumsfaktor, der für das abnorme Blutgefäßwachstum verantwortlich ist. Bisher ist nur ein Präparat, das von QLT (Toronto:QLT.TO) entwickelte Visudyne, zugelassen. Visudyne ist eine photodynamische Therapie, der Wirkstoff wird ins Blut injiziert und sammelt sich im Auge an, wo er mittels eines Laserstrahls aktiviert wird. Macugen wäre hinter Visudyne das zweite erfolgreiche Produkt gegen AMD am Markt, und ein großer Konkurrent für QLT, weshalb die Aktie des kanadischen Unternehmens am gestrigen Handelstag mehr als 8% an Wert verlor. Allerdings arbeiten auch bereits Genentech (NYSE:DNA), Genaera Corp. (NasdaqSC:GENR) und Alcon Inc. (NYSE:ACL) an Therapien gegen die altersbedingte Maculadegeneration (AMD).

Die Biotechbranche zählt zu den chancen- und zugleich risikoreichsten Sektoren, nur wer sich dieses Dualismus bewusst ist, wird langfristig Gewinne mit Biotechaktien erzielen können.

QLT (Nasdaq: QLTI) achieved what is still a pretty rare feat in the drug industry: attaining profitability through the performance of a drug. Visudyne is marketed by QLT's partner Novartis (NYSE: NVS) for the treatment of age-related macular degeneration (AMD). AMD is the most common cause of vision loss in people older than 50.


In 2003, Visudyne had $357 million in sales, of which $142 million was reported by QLT. Guidance for 2004 is $430 million to $455 million of Visudyne sales, and QLT expects to receive 30.3% of that amount plus reimbursement for certain costs.


Visudyne is an important drug for QLT as it is currently the company's only approved product. With competitors on the horizon looking to take market share, QLT's share price has come under pressure and is down 50% from its high of the past year.


The biggest near-term threat is Macugen from Eyetech (Nasdaq: EYET) and Pfizer (NYSE: PFE). Macugen just went before the FDA's Dermatologic and Ophthalmic Drugs Advisory Committee. While the committee did not take a vote, Macugen does have priority-review status, and I would venture a guess that it will be approved.


Over the next six months, biotech investors should keep an eye on QLT. If the share price continues to get pounded on concerns over Visudyne losing market share, the opportunity may present itself to pick up a good value. With the pending acquisition of Atrix Laboratories (Nasdaq: ATRX), QLT is bringing in some solid drug programs that could slip in under the radar as the market is distracted by the unfolding AMD situation.


Of particular interest is the six-month formulation of Eligard for the treatment of prostate cancer. If approved, as I expect, in the coming months, this will be the first testosterone-lowering drug of this duration on the market. With first-mover status, I think the sales could be substantial.
